Testing Request Form
  1. Read the Sales Terms and Conditions document.
  2. Download the appropriate form that matches the material being submitted for analysis:
  3. Complete the Testing Request Form.
  4. Ensure that the appropriate sample size is provided (see chart below).
  5. E-mail all testing requests to testing.request@precilab.com.
  6. A copy of the SDS (electronic preferred) is required for first submissions of all non-standard
    semiconductor materials. No work can be started without the SDS.

Bacteria testing for water samples will require sterile bottles.
TOC testing will require TOC certified glass vials.
If Fluoride is needed for Anion testing, we recommend to submit sample in HDPE bottle not Teflon bottle.

Requesting a Quote

For a request for quote, please e-mail as much detail about the analyte and matrix to be tested to sales@precilab.com. If email is not available, please contact Yves Gouanvic directly at phone number 972-661-8160 Ext 300.

The general information necessary for a quote is as follows:

  1. Chemical or material to be tested (an electronic copy of the SDS would be preferred for non-standard semiconductor materials).
  2. Type of analysis needed.
    • Specific elements or anions (see Trace Element List for current standard list of elements).
    • Number of assay components (with relative concentrations where possible) for multiple component materials.
    • Any customer specific details about the analysis required.
  3. Customer contact information (name, email, phone number, billing information, etc.)
  4. Number of samples and their frequency (one time project or if they will be submitted routinely).
  5. Describe any special experimental conditions required (assays at temperature, sampling to occur over 32 hour period, etc).
  6. Specify the analysis cycle time required (rush < 8 hours, next working day, weekend or holiday coverage).
  7. Specify how the quote should be communicated back (email, fax).

Note #1: Trace Element analysis size assumes having to analyze the sample for the routine 36 element list. 
Note #2: Anion analysis size assumes having to run the sample for the standard anions. 
** If necessary, smaller sample sizes can be analyzed. Please contact PRECILAB to verify sample size required.
